Now Hiring:Family Advocate & Resource Specialist, Care Coordination Program
As a key member of our team, your primary responsibility is to work collaboratively in the care coordination multidisciplinary team to support families of children with medical complexity. Providing social work services and consultation to families. You will utilize multiple health technology and communication platforms to support gathering information and communicating with families. This position will work closely with the Pediatric Nurse Navigator, Pediatric Providers, and other leadership for the Care Coordination program and will report directly to the Chief Operations Officer. This role is funded by a federal grant designed to improve the systems of care that we have for children with medical complexity in Montana. You will be part of serving patients as well as working to continuously understand the impact of your work and how the team can evolve the care model to be a meaningful, sustainable contribution to the lives of the children and the families that we serve.

- This position is part-time, up to 28 hours per week.
- Montana Pediatrics has an office location in Kalispell, however, telecommuting can be available for the right candidate.
